No-Shows

AlyneMD provides medical, aesthetic, longevity, and related services. Certain services may be elective, non-covered, or non-insurance-reimbursable.

For Payment

We may use and disclose PHI to obtain payment for services provided, including billing, claims processing, eligibility verification, utilization review, and collection activities.

For Health Care Operations

We may use and disclose PHI for health care operations, including quality assessment, credentialing, training, licensing, audits, administrative activities, and business planning.

Uses and Disclosures Without Authorization

We may use or disclose PHI without your authorization where permitted or required by law, including for:
Public health activities
Health oversight activities
Law enforcement purposes
Judicial or administrative proceedings
Coroners, medical examiners, and funeral directors
Organ and tissue donation
Research, subject to regulatory safeguards
Serious threats to health or safety
Workers’ compensation
Military, national security, or protective services

Uses and Disclosures Without Authorization

We may use or disclose PHI without your authorization where permitted or required by law, including for:
Marketing purposes where required by law

No Physician–Patient Relationship via Website

Use of this website, submission of inquiries, or receipt of non-clinical communications does not establish a physician–patient relationship. A physician–patient relationship is established only after a formal consultation with a licensed provider and completion of required consent documentation.

State-Specific Privacy Rights

Certain states provide additional protections for sensitive health information. Where state law provides greater privacy protections than federal HIPAA, state law governs.

New Jersey

Mental health records, substance use disorder records, HIV/AIDS-related information, genetic testing information, and sexually transmitted infection records may be subject to heightened confidentiality protections. Disclosure may require specific patient authorization except as otherwise permitted or required by law.

Pennsylvania

Mental health treatment records, drug and alcohol treatment records, HIV-related information, and certain genetic information are subject to additional confidentiality protections and may require explicit patient authorization.

Michigan

Mental health records, substance use disorder treatment records, HIV-related information, and genetic testing information may be subject to enhanced privacy protections and additional authorization requirements.

Florida

Mental health records, substance use disorder treatment records, HIV-related information, and genetic testing information may be subject to enhanced privacy protections and additional authorization requirements.

State Law Savings Clause

Mental health records, substance use disorder treatment records, HIV-related information, and genetic testing information may be subject to enhanced privacy protections and additional authorization requirements.
